Just found that D3 is wrong on the BOM here too - should be a 1N4148. Doesn't change anything for me unfortunately

I have the same problem too - I also bought the pcb from Synth DIY in Canada. I'd used the wrong resistors as noted here so I switched them today - still no effect and just the level pot working, the 3pdt I'm using as a switch does nothing either
